A tendon can make everyday life feel surprisingly small. Gripping a kettle with tennis elbow, climbing stairs with an aching Achilles, or reaching into a cupboard with shoulder pain can become a calculation. Tendinopathy PRP treatment may be considered when tendon pain has persisted despite sensible rehabilitation, activity modification and other appropriate care – but it is not a shortcut or a one-size-fits-all injection.

The right plan begins by understanding the tendon involved, the nature of the damage or irritation, and the activities you want to return to. For some people, platelet-rich plasma (PRP) can be a useful part of a wider recovery programme. For others, a more focused rehabilitation plan, shockwave therapy, or a different treatment pathway may offer a better starting point.

What is tendinopathy?

Tendons are strong bands of tissue that connect muscle to bone. They transfer force when you walk, lift, push, pull or play sport. Tendinopathy describes a tendon that has become painful and less able to tolerate load. It is often linked to a change in activity, repeated strain, reduced strength, biomechanics, age-related tissue change, or a combination of these factors.

The term is often used instead of “tendonitis”. This matters because long-standing tendon pain is not always driven by inflammation alone. A persistent problem may involve changes in tendon structure, sensitivity and load tolerance. That is why simply resting until the pain settles can be frustrating: a tendon usually needs carefully dosed loading to regain capacity.

Common areas include the Achilles tendon, patellar tendon below the kneecap, gluteal tendons at the side of the hip, rotator cuff tendons in the shoulder, and the common extensor tendon associated with tennis elbow. Symptoms may be worse on first moving after rest, during a familiar activity, or later that day after doing too much.

How tendinopathy PRP treatment works

PRP is prepared from a small sample of your own blood. The sample is processed to concentrate platelets in plasma, then injected into or around the affected tendon region. Platelets contain signalling proteins that are involved in the body’s repair response. The aim is to influence the local healing environment and, alongside rehabilitation, support a gradual improvement in pain and function.

Who may benefit from PRP for tendinopathy?

That commitment is crucial. An injection may create an opportunity for recovery, but it does not rebuild tendon capacity by itself. The lasting goal is not simply a quieter tendon on the scan or a few pain-free days. It is being able to walk further, return to the gym, work comfortably, play with grandchildren, or resume the sport you enjoy with confidence.

PRP is not always the best choice. A substantial tendon tear, a problem arising mainly from a joint, referred pain from the spine, a nerve issue, or an inflammatory rheumatological condition may need a different approach. Equally, a tendon that has not yet had an appropriate period of targeted strengthening may improve without an injection.

Your medical history matters too. Medication, bleeding risk, active infection, pregnancy, significant health conditions and previous treatments all need discussion. Do not stop prescribed medicines, including anti-inflammatory or blood-thinning medication, without individual medical advice.

What does the evidence say?

Research into PRP for tendon disorders is encouraging in some areas, but it is not uniform. Studies use different PRP preparations, injection techniques, rehabilitation programmes and outcome measures. They also assess different tendons, at different stages of tendinopathy. This makes broad promises inappropriate.

Some patients experience meaningful improvement in pain and function over the following weeks and months. Others notice little difference. Results can depend on the tendon involved, the duration and severity of symptoms, the accuracy of diagnosis, your general health, and how consistently rehabilitation is completed.

A realistic conversation should therefore include both potential benefit and uncertainty. PRP is generally used as part of evidence-informed, individualised care rather than as a guaranteed cure. A clinician should be able to explain why they believe it may suit your particular presentation, what alternatives are available, and how success will be measured.

The rehabilitation that follows matters most

Tendons respond to load, but they need the right dose. After a PRP injection, your rehabilitation programme is usually phased according to the tendon treated, your symptoms and your functional goals. Early management may focus on settling post-procedure discomfort and maintaining gentle movement. This progresses into carefully selected strengthening work, then more demanding functional or sporting tasks where appropriate.

For Achilles tendinopathy, that may mean progressing calf strength and walking tolerance before returning to hills, running or court sports. For gluteal tendinopathy, it may involve improving hip strength while reducing positions that compress the tendon, such as prolonged side-lying or crossing the legs. With tennis elbow, grip, wrist and forearm loading are often rebuilt in a structured way.

Pain during rehabilitation is not always a sign of harm. Tendon exercise can be challenging, and a degree of manageable discomfort may be acceptable. What matters is the overall response: whether symptoms settle appropriately, function is improving, and load can gradually increase. Your physiotherapist can help you distinguish useful training feedback from a flare that needs adjustment.

What to expect from a PRP appointment

A specialist appointment should not feel rushed. It begins with listening to your history: when the pain started, what aggravates it, previous injuries, your work demands, exercise habits and what you have already tried. A physical examination assesses movement, strength and the structures that may be contributing.

Where indicated, point-of-care ultrasound provides further information during the consultation. If PRP is appropriate and you choose to proceed, blood is taken and processed before the injection. The procedure is performed using sterile technique, with ultrasound guidance where clinically indicated.

You should leave with clear aftercare advice rather than a vague instruction to “rest”. This includes guidance on pain relief, relative activity modification, the likely recovery timeline, when exercise should restart, and when to contact the clinic. Follow-up allows the plan to respond to your progress, not just the calendar.

When to seek assessment sooner

Not all tendon pain should be managed as routine tendinopathy. Sudden loss of strength, a popping sensation, marked swelling or bruising, inability to bear weight, a hot red joint, fever, or pain that is severe and unexplained needs prompt clinical assessment. These symptoms can indicate a tear, infection or another condition requiring a different level of care.

Persistent pain also deserves proper assessment when it is affecting sleep, work, mobility or mood. Waiting until you can no longer do the things that matter to you often makes recovery feel more daunting than it needs to be.
