A painful knee on the stairs, a shoulder that interrupts sleep or a tendon that will not settle can make every decision feel urgent. When considering PRP versus steroid injections, the most useful question is rarely which treatment is ‘best’ in general. It is which option suits the structure causing your pain, the stage of your condition and the activity you want to return to.
Both treatments can have a place in musculoskeletal care, but they work in very different ways. A careful clinical assessment, often supported by musculoskeletal ultrasound, helps distinguish joint inflammation from tendon disease, arthritis from a tear, and pain that may respond well to an injection from pain that needs a different approach altogether.
PRP versus steroid injections: the key difference
A corticosteroid injection is an anti-inflammatory treatment. It aims to reduce inflammation and pain, sometimes quite quickly. Steroid injections are commonly used where there is an inflammatory flare in or around a joint, bursa or tendon sheath, such as a painful arthritic joint, shoulder bursitis or trigger finger.
Platelet-rich plasma, usually called PRP, is made from a small sample of your own blood. The blood is processed to concentrate platelets, which contain signalling proteins involved in the body’s repair response. The PRP is then injected into the area being treated, often under ultrasound guidance.
PRP is not a painkiller and does not usually give the rapid response some people experience after a steroid injection. Instead, it is intended to influence the local healing environment. This makes it a consideration for selected tendon problems and some joint conditions, particularly when the goal is to support longer-term improvement alongside a structured rehabilitation plan.
When might a steroid injection be the better choice?
Steroid can be valuable when inflammation is the main driver of pain and is preventing you from moving, sleeping or engaging with rehabilitation. For someone with a painful flare of knee osteoarthritis, inflammation around the shoulder, or a swollen and irritable joint, reducing pain may create a useful window to restore movement and strengthen the supporting muscles.
The benefit can begin within days, although it may take up to a couple of weeks to judge the full effect. Relief is variable. Some patients have significant improvement for several months, while for others it is more modest or short-lived.
Steroid injections are not simply a quick fix to repeat whenever pain returns. Repeated injections into the same area may not be appropriate, particularly around certain tendons where steroid can affect tissue quality. In a weight-bearing joint, the decision also needs to account for the degree of arthritis, previous injections, future treatment plans and your wider health.
People with diabetes should be aware that corticosteroid can temporarily raise blood glucose levels. Other considerations include active infection, certain medicines, planned surgery and previous reactions to injection treatments. These issues should be discussed before proceeding.
Where can PRP have a role?
PRP is often considered for persistent tendon conditions, including some cases of tennis elbow, Achilles tendinopathy, gluteal tendon pain around the hip and patellar tendon pain. It may also be discussed for selected people with knee osteoarthritis, especially where symptoms remain limiting despite appropriate exercise, activity modification and simpler treatments.
The evidence for PRP is promising in some areas but is not identical for every condition. Results depend on the diagnosis, the severity and duration of symptoms, the preparation used, the accuracy of injection placement and the rehabilitation that follows. PRP should therefore not be presented as a guaranteed tissue-regeneration treatment.
After PRP, it is common for the treated area to feel more sore for several days. Improvement is generally gradual, often becoming clearer over weeks rather than days. This slower timetable can be frustrating, but it is relevant for people hoping to reduce reliance on repeated anti-inflammatory injections and who are able to commit to a progressive recovery programme.
Because PRP uses your own blood, allergy is not usually the central concern. It is still an injection, however, so temporary pain, bruising, bleeding and infection are possible risks. Suitability can be affected by blood-thinning medication, low platelet levels, active infection and some medical conditions.
The condition matters more than the trend
It is easy to compare treatments as though every painful joint or tendon behaves the same way. They do not. A steroid injection may be appropriate for an inflamed bursa but less suitable for a degenerative tendon. PRP may be worth considering for a longstanding tendon disorder but may not address pain caused by a significant tear, nerve irritation or advanced joint change.
This is why diagnosis comes first. A consultation should explore where the pain is, what loads trigger it, whether there has been injury, how it affects sleep and function, and what treatments you have already tried. Musculoskeletal ultrasound can add valuable information by showing features such as fluid, inflammation, tendon thickening, calcification, tears and joint changes in real time.
Ultrasound guidance can also improve injection precision by allowing the clinician to see the target structure and avoid nearby nerves, blood vessels and vulnerable tissue. It is especially useful when treating smaller joints, tendon sheaths, bursae and deeper structures.
Recovery does not end with the injection
Whether you choose PRP or steroid, the injection should serve a wider recovery goal: walking with less discomfort, returning to work, sleeping through the night, getting back to the gym or being able to enjoy a valued hobby.
With steroid, the period of reduced pain is an opportunity to address the factors that contributed to the problem. That may mean improving joint range, building strength around an arthritic knee, altering training load or correcting a movement pattern that repeatedly aggravates a tendon.
With PRP, rehabilitation is equally central. Tendons respond to carefully graded loading, not complete rest indefinitely. Your clinician may recommend a short period of relative rest after treatment, followed by a progressive programme tailored to the tendon, your symptoms and the demands of your daily life. Returning to running, racquet sports or heavy lifting too quickly can undermine recovery regardless of the injection chosen.
How to decide between PRP and steroid injections
A good decision balances the likely source of pain, the speed of relief you need, your medical history and your preferred recovery pathway. There are occasions when a steroid injection is a sensible, evidence-based way to settle a disabling flare. There are others when PRP may better align with a gradual strategy for persistent tendon or joint symptoms.
It may also be appropriate to choose neither. Exercise-based physiotherapy, advice on pacing, weight management where relevant, shockwave therapy for selected tendinopathies, hyaluronic acid or other joint treatments may be considered depending on the diagnosis. Surgery is occasionally necessary, but many musculoskeletal problems can be managed without it when the treatment plan is specific and timely.
