A musculoskeletal ultrasound scan guide should begin with one reassuring fact: this is a dynamic, radiation-free examination designed to help make sense of pain, swelling, weakness or reduced movement. Rather than relying on a picture alone, the scan is most useful when it is part of a careful clinical assessment – listening to your symptoms, examining the area and relating what is seen to the activities you want to return to.
For someone who has struggled to lift an arm, walk comfortably, grip a kettle or get back to sport, a clear explanation can be as valuable as the scan itself. Ultrasound can often provide useful answers quickly, while also helping to plan the right next step.
What is a musculoskeletal ultrasound scan?
Musculoskeletal ultrasound uses high-frequency sound waves to create live images of structures close to the skin. These include tendons, muscles, ligaments, bursae, joints, nerves and areas of fluid or inflammation. A small handheld probe is moved across the skin with gel, producing images on a screen in real time.
Unlike an X-ray, ultrasound does not use radiation. Unlike an MRI scan, it is usually performed while you are awake and can move the affected body part. This matters because many musculoskeletal problems only become apparent during movement. A shoulder tendon can be viewed while you lift your arm; a tendon at the ankle can be assessed as the foot moves; and a nerve may be examined along its course.
It is not a replacement for every type of imaging. X-rays remain useful for assessing bones, joint alignment and established arthritis changes. MRI may be more appropriate for deeper structures, complex spinal concerns, certain cartilage injuries or when a broader view of a region is needed. The best investigation depends on the question being asked, not simply on which scan is available.
When can an ultrasound scan help?
Ultrasound is particularly valuable when symptoms suggest a problem in a soft tissue structure or a superficial joint. It can help investigate persistent pain, a new lump or swelling, a suspected tear, reduced movement after an injury, or symptoms that have not settled as expected with initial care.
Common reasons for a scan include shoulder pain from possible rotator cuff tendinopathy, bursitis or calcium deposits; tennis or golfer’s elbow; wrist and thumb tendon pain; hip bursitis; knee swelling; Achilles or plantar fascia pain; and tendon problems around the ankle and foot. It can also assist assessment of some nerve-related symptoms, such as nerve thickening or compression around the wrist.
For people with inflammatory arthritis or suspected joint inflammation, ultrasound may identify synovitis, which is inflammation of the joint lining, or increased blood flow associated with active inflammation. It can also show fluid within a joint or bursa. These findings need to be interpreted alongside your history, examination and, where relevant, blood tests or rheumatology care.
What can a musculoskeletal ultrasound scan show?
A scan may show tendon thickening, small or larger tears, calcification, inflammation around a tendon, fluid in a bursa, muscle injury, ligament disruption or joint effusion. It can also identify changes linked with osteoarthritis, such as small bony spurs at the joint margins, although X-ray is often better for showing the full bony picture.
The live nature of ultrasound is a major advantage. Your clinician can compare the painful side with the other side, press gently over the structure that reproduces your familiar pain and watch what happens as you move. This can help distinguish an incidental finding from one that is genuinely relevant.
That distinction is essential. Scans sometimes show age-related tendon or joint changes in people with no pain at all. Equally, a scan can look relatively reassuring even when pain is significant, because pain may be driven by load sensitivity, joint stiffness, nerve irritation or altered movement patterns rather than a visible tear. A result is therefore not a verdict on what you can or cannot do.
Preparing for your appointment
Most musculoskeletal ultrasound appointments require very little preparation. Wear loose clothing that allows the painful area to be exposed easily. For a shoulder scan, a vest top is helpful; for a knee, hip or ankle assessment, shorts or loose trousers can make the examination more comfortable.
Bring any previous scan reports, hospital letters and a list of medicines if you have them. It is also useful to think about what brings your symptoms on, what eases them and which movements or activities you are currently avoiding. If you have had an injection, operation or recent injury, mention when it happened.
You can generally eat and drink normally before a routine musculoskeletal ultrasound. If the scan may lead to an ultrasound-guided injection on the same day, your clinician will explain any specific advice in advance, including medicines that may affect bleeding risk.
What happens during the scan?
The appointment should start with questions, not gel. A clinician will ask where your pain is, how long it has been present, how it affects daily life and whether there has been trauma, swelling, morning stiffness, numbness, weakness or locking. They will then examine movement, strength and tender areas.
You will be positioned to allow access to the body part being assessed. A small amount of water-based gel is applied to the skin, then the probe is moved over the area. The gel can feel cool, and gentle pressure may be needed to obtain a clear image. Tell the clinician if a position is painful – the examination can often be adapted.
A typical focused scan may take 15 to 30 minutes, although a combined consultation, examination and treatment-planning appointment can take longer. If the source of your symptoms is identified, the clinician may use the images to explain the finding as you look at the screen together.
How results guide treatment and recovery
The most helpful outcome is not merely a list of scan findings. It is a practical plan that reflects the tissue involved, the severity of symptoms, your health history and your goals. A small tendon tear in a retired gardener and the same finding in a manual worker may require different decisions because their demands are different.
For many tendon, muscle and joint problems, rehabilitation remains central. This may include graded strengthening, improving movement control, pacing activity, hands-on physiotherapy, strapping or a targeted home exercise programme. The aim is to build capacity so that the affected area can tolerate the walking, work, exercise or hobbies that matter to you.
Where inflammation is contributing to pain, medication advice or an injection may be considered. Ultrasound guidance can improve precision when treating a joint, bursa, tendon sheath or other targeted structure. Depending on the diagnosis and suitability, options may include corticosteroid, hyaluronic acid, platelet-rich plasma or other interventional pain treatments. No injection is automatically the right answer: benefits, limitations, recovery time and alternatives should be discussed clearly before you decide.

Questions worth asking after your scan
Before you leave, make sure you understand what was found and what it means in your case. Ask whether the scan finding is likely to explain your pain, whether there are activities you should temporarily modify, and what you can continue doing safely. It is also reasonable to ask whether further imaging, a medical review or a rheumatology opinion is needed.
If treatment is recommended, ask what improvement should realistically look like, how long it may take and what would prompt a change of plan. Recovery is rarely perfectly linear. Knowing what is expected can make temporary fluctuations feel less alarming and help you stay engaged with rehabilitation.
When a scan should not delay urgent care
Ultrasound is not the first step for every painful problem. Seek urgent medical advice for a hot, red, rapidly swollen joint, fever with joint pain, sudden inability to bear weight after injury, a visibly deformed limb, new severe weakness, or numbness around the groin with changes to bladder or bowel control. These symptoms may need prompt assessment in a different setting.
For ongoing pain or loss of confidence in movement, a well-timed scan can replace uncertainty with a clearer route forward. The goal is not simply to name a structure on a screen, but to help you make informed decisions and move towards the life you value with greater comfort and confidence.
